About Healthier SG
Healthier SG is a Government initiative by MOH with its main focus on preventive health.
It hopes to encourage patients to build a long term trusted relationship with their primary care provider.
This includes a personalised Health Plan which includes screening tests and vaccinations.
Your first Health plan discussion and annual check-in are fully subsidised.
CHAS, Merdeka or Pioneer Generation cardholders with high chronic medication bills that exceed the annual CHAS subsidy limits, can now switch from their CHAS tier to the Healthier SG tier (HSG tier) for chronic diseases at their enrolled Healthier SG GP clinic, at prices that are comparable to those at polyclinics.
How to Enrol
Singapore Citizens and Permanent Residents aged 40 years old and above can enrol in Healthier SG using the HealthHub app.
If you have not received the SMS invite, you may request for the SMS invitation by submitting your details at https://go.gov.sg/healthiersg-sms-invite, or call MOH hotline at 63259220.
What is a health plan?
A Health plan is a personalised plan developed after discussion with your Primary Care Doctor after going through your risk factors and state of health.
All Health plans are free of charge and enrollees can receive $20 worth of Healthpoints after your first Health Plan consultation. Your Doctor will also advise you on the vaccinations that you are eligible for at a fully/partially subsidized rate, as well as your free chronic and cancer screenings (subject to eligibility)Benefits
Eligible Singapore Citizens benefit from subsidies for nationally-recommended vaccinations listed on the Subsidised Vaccine List as well as free chronic and cancer screenings based on their age and risk factors.
